Itinatag sa himpilan ng hukbo ni Heneral Licerio Geronimo sa Bundok ng Puray sa bayan ng Montalban, 13 Mayo 1897. Pinangasiwaan nito ang mga pamahalaang rebolusyonaryo sa mga lalawigan ng Maynila, Bulacan, Pampanga, Bataan, Nueva Ecija, Laguna at Morong. Pinamunuan nina Padre Pedro Dandan, Pangulo; Dr. Anastacio Francisco, Pangalawang-Pangulo; Teodoro Gonzales, Kalihim ng Pamahalaan; Cipriano Pacheco, Kalihim ng Digma; Paciano Rizal, Kalihim ng Pananalapi; at Feliciano Jocson, Kalihim ng Gawang-Pagpapaunlad.
